Scheduling An Appointment

Appointments can be made in two ways:

•   If you are an established patient you will be given access to your EHR (electronic health record) portal.  In your portal, you will be able to schedule appointments and view your records. You can also text or email Auz. 
• If you are new, go to the Contact Auz link and send me an email.  I will reply and we will set up your initial appointment. Please include your date of birth.
